Here are 3 examples of work summaries. Remember, your work summaries are a brief descriptive account of any major pieces of work you undertook during your placement that you intend to analyse or reflect upon in your Critical Self Evaluation. They provide the background. They are not the section of your Portfolio where you make links to theory / knowledge / law / policies / procedures etc.
Child E and Child F
E is a 2 year old, white British girl and her brother, F, is a 1 year old, white British boy. Both children were placed in foster care in March 2015 due to concerns about their birth parents ability to provide appropriate care. After a series of court hearings and intensive parenting assessments, the children have been placed with their prospective adopters. I have been co-working the case and have taken responsibility for the statutory LAC visits and LAC reviews. I was originally visiting the children at their foster home every 4 weeks and, since they became placed for adoption, have been visiting weekly. Visits included observing and assessing the children’s health, well-being and development. I have been responsible for maintaining records of the visits, sharing and gathering information between agencies and professionals. I have undertaken direct work with the children around introductions to their adoptive parents, using stories drawings and photographs, have helped complete their life story books, meeting with the children’s birth father and foster carer, as well as gathering information from the children’s files. I have attended pre and post adoption panel meetings, contributing to discussions and decision making.
Mrs V is an 83 year old Black British woman, who prior to her retirement in 2003 had worked as a Head Teacher in an inner London primary school. After the death of her husband in 2007, she was left without any immediate family or social support networks. Over the past year, she had repeatedly declined offers from social services and occupational health for assessment and possible social care support, considering herself to be entirely independent and in excellent health. Neighbours and local police made a series of referrals after Mrs V had begun to sleep in the corridor of her apartment building and exhibit signs of physical and mental frailty. I conducted an initial assessment. Focusing on Mrs V’s ability to protect herself from harm. I worked alongside a range of other professionals to consider the risks and to assess Mrs V’s mental capacity. I took part in a Best Interests assessment and worked to support Mrs V’s transition into sheltered accommodation. I advocated on her behalf, in order that she was placed locally within the community she knew well.
Children’s teams A, B and C share responsibility for covering duty in the borough equally across the teams. In the absence of the allocated social worker, the duty social worker is expected to, with supervision from the duty manager, manage all general incoming calls for the Looked After Children service, forward messages, manage crises that may occur and manage disrupted contact arrangements. The duty social worker records tasks undertaken and updates allocated social workers upon their return from absence. I spent regular periods throughout my placement covering the duty social worker role.
